Running an assessment
Complete questions and manage evidence
Record answers, explain uncertainty and connect supporting evidence to the relevant controls.

Before you start
Open the correct assessment and confirm its scope. Administrators and editors can answer controls when the assessment is available for editing. A finalised assessment or one awaiting approval may be protected from changes.
Answer a control
Open a control and read its question and guidance.
Choose the answer that reflects the current situation. Use the descriptions beside the options. Questions may use a 0 to 5 scale or Yes and No.
Answer the evidence question separately. How well a practice works and how well you can support the answer are different things.
If the control permits Unknown or Not applicable, use the appropriate option. Add a reason when requested. Do not use these options simply to avoid a difficult question.
Wait for the answer to save before moving on. If saving fails, keep the page open and resolve the error.
Add useful evidence
Use the evidence controls beside the question to record supporting references or notes. Explain which part of the source supports the answer. Use a clear name so another reviewer can identify it.
Check the Evidence area to review sources across the assessment. A reference alone does not show that the evidence has been checked. Review its relevance, date and access before relying on it.
Check your work
Reopen a completed question to confirm the saved answer. Check any missing information and review warnings before finalisation. Avoid putting passwords or unrelated personal information in answers or notes.
